Online Registration Form - Resume receiving credit card statement by mail for selected customers

As an advocate of environmental protection, Dah Sing Bank ("the Bank") has always encouraged our customers, especially the e-Banking Service customers, to make good use of electronic channel in order to reduce paper consumption. To this end, we are now providing our service of credit card e-Statement and suppressing credit card statement by mail ("Credit Card e-Statement Service") to customers by phases. As a selected customer, you can enjoy this service without the need to go through the registration process. With the use of such service, you can gain access to your credit card e-Statement in a timely manner as well as contribute to save our planet.

Starting from 28th December, 2017, you will be automatically enrolled in our Credit Card e-Statement Service. From then onwards, you will be receiving a free email notification to remind you to login to our e-Banking / Mobile Banking Services to review your credit card statement when your latest e-Statement is issued.

If you wish to continue receiving credit card statements by mail after the Effective Date, please fill in and submit the below information on or before 21st December, 2017.

    > If you have more than one Dah Sing Credit Card account, you may enter any one of your Principal Credit Card account numbers.
HKID No.
(the last 4 characters including the character featured in brackets)		 XXXX
    > Please enter the last 4 characters (including the character featured in brackets) of HKID corresponding to the above Credit Card Account No.
    Example 1: if your HKID No. is A123456(3), please enter 4563.
    Example 2: if your HKID No. is A456789(A), please enter 789A.

Remarks:
  1. This registration service is only applicable to selected customers and principal credit card cardholders.
  2. If you have suppressed receiving statement by mail at "e-Statement & Advice / e-Alert" > "e-Statement & e-Advice / e-Alert Settings" in e-Banking / Mobile Banking Services before the Effective Date, this resume receiving credit card statement by mail registration ("Registration") shall resume your setting of receiving statement by mail on the Effective Date.
  3. Starting from the Effective Date, this Registration will be applicable to all Dah Sing Credit Card account(s) (including supplementary card accounts) and new Credit Card account(s) you open in the future under your account name.
  4. If you wish to resume or suppress receiving credit card statement by mail after 28th December, 2017, please make your amendment at "e-Statement & Advice / e-Alert" > "e-Statement & e-Advice / e-Alert Settings" in e-Banking / Mobile Banking Services.
  5. Your information provided will be encrypted during transmission.
  6. The Bank will use the collected personal data (including the credit card account number and the last 4 characters of HKID) for verification purpose and such personal data will be destroyed within 1 year from the date of registration on resuming to receive credit card statement by mail.
