Remarks: Promotion Period of YOU Banking Reward is from 1 April 2020 to 30 September 2020 (both dates inclusive) ("Promotion Period"). This registration form is applicable to the eligible customers of YOU Banking Reward and referrers of YOU Banking Referral Program ("Customers"). If a Customer chooses Avios as YOU Banking Reward, the Customer is required to register through this form during the registration period as stipulated below ("Registration Period"): (a) Eligible customers of YOU Banking Reward: within 3 calendar months from the Account Opening Date (b) Referrers of YOU Banking Referral Program: from 1 April 2020 until the 3rd calendar month from the Account Opening Date of the first referee Once the registration is submitted, no amendment or cancellation can be made. If the Customer does not have a valid Dah Sing British Airways Platinum Card Principal Card at the time the Avios is credited, or if our Bank has not received any registration record during the Registration Period, the Customer will be deemed to have chosen the Credit Card Cash Reward as the YOU Banking Reward. Customers are required to register successfully during the Registration Period and fulfill relevant requirements in order to enjoy the relevant YOU Banking Reward. Upon successful registration, Customers will be issued a reference number. Please mark down it for future reference as it will not be reissued. If a Customer has submitted wrong information, the registration will be regarded as invalid without further notification. The promotion is subject to relevant terms and conditions.
